By booking or attending an appointment with our clinic, you agree to the following Terms of Service:
1. Payment for Services
Payment is due on the day services are provided. In most cases, payment is collected prior to the start of your appointment
2. Accepted Payment Methods
-Cash-Credit cards
-Debit cards
-Interac e-Transfers (Payment must be successfully processed before or at the time of service.)
-Direct billing of Insurance (see below)
3. Insurance & Direct Billing
Encompass Health & Wellness offers direct billing to most major insurance providers, when available. Some insurance plans or providers may not be eligible for direct billing. It is the client/patient’s responsibility to:
-Confirm coverage and eligibility with their insurance provider prior to the appointment; --Pay any portion not covered by insurance, including co-payments, deductibles, or denied claims.
If an insurance claim is declined or partially paid, the outstanding balance becomes the patient’s responsibility and must be paid the day of the appointment.
4. Outstanding Balances
Any agreed upon unpaid balances must be settled before future appointments may be booked. We reserve the right to charge the credit card on file for outstanding amounts.
5. Appointment Attendance
Patients are expected to attend appointments on time. Late arrivals may result in shortened appointment times while still being charged the full service fee.
6. Cancellation Policy
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists' day that could have been filled by another patient. As such, we require 48 hours notice for any cancellations or changes to your appointment. Patients who provide less than 48 hours notice, or miss their appointment, will be charged a cancellation fee to the card on file.
7. Changes to These Terms
Encompass Health & Wellness reserves the right to update or modify these Terms of Service at any time. Updated terms will be made available in-clinic and/or on our website.
